Fill out this form to complete your registration for Downtown Menomonie’s Golf Scramble on June 11th. Lunch selection will be sent via email at least one week before the scramble.

To register, add preferred items to your cart. Individual and team registrations will have a pop-up form for you to input the golfer's information. When you are ready to check out, click the cart icon on the bottom right of your screen and complete the process.
